Discharge Challenge Project

The Region IV Mental Health Directors developed an innovative program that focuses on Central State Hospital consumers who are clinically ready for discharge, but who are particularly anxious about leaving the facility to live independently.  This Project will target Adult Living Facilities (ALFs) throughout the region and provide incentives for them to provide a welcoming placement for this group of consumers.  Staff from the ALFs will also be expected to begin forming a relationship with the consumers prior to their discharge from CSH.  In addition, peer specialists from VOCAL  will serve as mentors for the consumers throughout the discharge process.  $110,000 in funds have been allocated for this purpose, with the hope of transitioning 10 consumers out of the state facility setting during 2007.

Enhancing public mental health in Central Virginia

The following projects remain in some part of the planning or implementation process in Region IV.

             A number of Region IV consumers currently hospitalized at Central State Hospital (CSH) have been identified as ready for discharge and in need of placement in local nursing home facilities. Unfortunately, CSH and the regional CSB/BHAs have been unable to locate local facilities that are willing to accept these individuals. The reluctance on the part of the local nursing homes is due primarily to concerns about the consumers' mental health behaviors/issues, and fiscal concerns based on the perception that these consumers would require enhanced staffing patterns in order to appropriately serve them in local facilities.

                 In response to the dilemma described above, Region IV Reinvestment specifically allocated funds to use to cover the cost of the enhanced services that a nursing home facility felt would be required to best serve one of these consumers. The local CSB/BHAs responsible for discharge planning for the consumers approached selected nursing homes, describe the specific consumers involved, and explained that the Region would provide a maximum of $20,000 per year/per consumer in Phase I funding to cover the cost of any additional staffing/services needed by the facility. (Phase II allocations would be $12,150 per consumer/per year).

                 After searching for several years for local facilities to serve the needs of these type of consumers, it was determined that there are no appropriate and willing providers within Region IV.  Therefore, a request for proposals went out asking providers to bid on host  homes to serve a group of these consumers coming out of CSH.  Region IV Reinvestment is currently planning for the opening of the first host homes in late 2007.

Region IV will be actively involved in the implementation of a regional homelessness initiative with Virginia Supportive Housing.  The program will follow the housing first model, offering intensive services to mentally ill persons experiencing chronic homelessness.
